Hannah Lemon
With 18 years of experience as a physiotherapist, Hannah has dedicated the last 9 years to community-based rehabilitation, supporting people to live safely, confidently, and independently in their own homes. She is passionate about helping individuals achieve their personal goals and believes that with the right support, people can continue to live happy, fulfilled lives with dignity and independence.
Hannah provides comprehensive assessments within people’s own homes, taking a holistic approach that considers the whole person and every aspect of their daily life — not just how they move. Following the initial assessment, she works closely with clients and, where appropriate, their families to establish meaningful goals and develop tailored plans to help achieve them.
This may include personalised exercise programmes, care plans, equipment advice, recommendations for home adaptations, massage, facilitated movement, onward referrals, and ongoing guided support. With her extensive experience, Hannah is able to adapt every session according to each client’s individual presentation, needs, and goals. She uses a varied and flexible approach to engage people effectively, recognising that every person responds differently and requires personalised support.
